Home and Community Based Services (HCBS) is a Medicaid program offering tailored, out-of-school services for eligible youth. A dedicated care manager works closely with the child and their family to outline a comprehensive Plan of Care (POC). The insurance company reviews and approves the child's assessment and Treatment Plan. Throughout the process, they may periodically check in on the child's progress or discuss any billing discrepancies to ensure the child receives the necessary support.
Families and caregivers work in partnership with the care manager to identify a qualified service provider who can deliver the prescribed and approved services for their child. This collaborative approach helps ensure that the child receives the most appropriate and beneficial support possible.
